TOURNAMENT APPLICATION

Application and Fee Deadline:
OCTOBER 5, 2010 Application Fee: $850

Application and fee must be received by October 5.
Print a copy of your completed application and confirmation, and mail both with your check 
(payable to "PDA", please note club and team name on check) to:

Please note that all checks will be cashed upon receipt. This is not an indication of acceptances. Refunds will be issued within 2 weeks of posting accepted teams. There will be no refunds once accepted.

REQUIRED REGISTRATION MATERIALS:

US Club Soccer Teams
1.  US Club Roster – original and 2 copies
2.  Valid US Club Player Passes
3.  Current US Club signed Medical Release for every player
